2026 CLARA BARTON CAMP & CAMP JOSLIN RESIDENT CAMPS CONFIRMATION FORMS
Submission of the following forms is required for camp attendance and must be completed at least two weeks prior to the session start date. These requirements are in addition to camp registration.
- Required Paperwork Checklist
- Camper Handbook– The Barton Center Camp Joslin, Clara Barton, Vermont Overnight
- Handbook Agreement
- Authorization to Release Form
- Current Insulin Plan
- Authorization to Administer Medication
- Self-Administration Form
- Resident Camper Intake Form
- Non-FDA Product Waiver (if applicable)
- Transportation Form (for summer resident camps CJ and CBC ONLY)
- Meningococcal Disease Information Notice
A copy of the following must be submitted to the Barton Center:
- Annual Physical Exam (valid within the last 12 months)
- Primary Care Provider Approval Form
- Immunization Records
- Camper’s Health Insurance Card & Prescription Card (front & back)
